AFFILIATED BUSINESS ARRANGEMENT NOTICE
This is to give you notice that each of the "referring party" selling entities (as more particularly identified in the applicable Real Estate Sales Contract for each of its respective jurisdictions (collectively, "Selling Entity") has a business relationship with MC Mortgage. Specifically, the Selling Entity (as defined above), or its affiliate, holds ownership of at least 1% in MC Mortgage. Because of this relationship, this referral may provide Antares Homes with a financial or other benefit.
Set forth below are the estimated charges or range of charges for the settlement services listed (these charges may vary according to the county or area in which the property is located). YOU ARE NOT REQUIRED TO USE MC Mortgage AS A CONDITION OF THE PURCHASE OF THE SUBJECT PROPERTY OR THE SETTLEMENT OF YOUR LOAN. FREQUENTLY THERE ARE OTHER SETTLEMENT SERVICE PROVIDERS AVAILABLE WITH SIMILAR SERVICES. YOU ARE FREE TO SHOP AROUND OR DETERMINE THAT YOU ARE RECEIVING THE BEST SERVICES AND THE BEST RATE FOR THESE SERVICES.

Upon entering into the Real Estate Sales Contract with the Selling Entity representative you will be provided a written Affiliated Business Arrangement Notice.
If you choose to apply with MC Mortgage, an initial disclosure of all settlement charges will be provided to you within three business days after completed loan application.
By clicking Accept below, I/we acknowledge we have read this disclosure form and understand the affiliation existing between the Selling Entity and MC Mortgage. I/we also understand that the Selling Entity, or its affiliates, may receive a financial or other benefit as the result of our utilizing the services of MC Mortgage.
